Excellent, I see where this can go!

Just two remarks for now:

1. can secure sites (https://*) be excluded from sidewiki by default? These are many times used for banking, webmail and such and that is not exactly where you want to load a widget like sidewiki (or anything else from a remote server for that matter).

2. Why not open the sidewiki link in the sama window/tab as the current page? That wat the page is displayed as before, only with the sidewiki content at the left. Much more natural! You only have to exclude the sidewiki entry urls:

Keep up the good work and I can't wait to use this script for commenting myself!

"https://*" removed from the sites included by default. You're absolutely right.
Target of a sidewiki link (it's the title of an annotation, next to the author)changed to _top (the window/tab of the annotated document). The target of "all annotations by the author" (the linked under author's name) set to "sidewiki-by-author". It's left to see how will this be liked.
The exclude does not seem to work. I added it anyway, but I think that because the annotated document is opened in a subframe, it cannot work. I'm blocking such urls in the code for a moment (with RegExp.test).
