Hello!
Because GFX uses url query to determine if image search results are filtered or not (safe mode) it will show the wrong "safe" icon at first page if safe mode disabled directly in google's settings.
Since google's cookies are encrypted, the only way I see this can be fixed is force &safe= in the url in all pages.
At initial search page (http://images.google.com) a hidden input can be append to the search form, on the rest search results google will add &safe= into url query automatically, which will override the settings in the cookies.
