Can someone explain passive refresh please?

If I choose yes, does that mean it will refresh every 1 minute or every 5 minutes?

Don't quote me, but I think...:

It refreshes every 1 minute when you have the stamina to hit someone on the hit list, or every 5 mins if you are out of stamina, until you have stamina to use again. A more passive version of the aggressive refresh you might sa.y :)

based on the fact that its been 5 hours and nobody has responded, I'm guessing nobody really knows exactly how it works. I'll have to wait for PC to fill me in.

It tries to hit someone on the hit list according to your hit criteria every minute while you have stamina. When you don't have stamina, it checks to see if you have regained any every 5 minutes.

It also is quite uesful if you level up by other people attacking you, as you set your health to x% and a refresh is done every 1 or 5 mins to keep you out of the hospital so you can be attacked...

Chrisos that's how I use it. I've built up my defenses such that I try to stay as healthy as possible so I can win more fights. When someone loses to me, I punch them in the face if they are still on line to entice them to hit me back :) Works about 50% of the time.

Yep, that's how I have levelled for the last 20-30 levels or so, let other people use their stamina to attack me, and even if they do win the fight, they can only hit me so often before they run out of stamina, and I am at most $5M down in direct losses (chump change) after 50 attacks, and 20M down after all the rehealing (four chump's worth of change).

It is only a problem if you are being repeatedly histlisted, as you will waste all the cash you have in the bank on hospital fees, to fund losing experience points. Hence I try not to start fights.

I just sit there looking weak (I'm not increasing my mob size at the moment) like a little victim with no friends, but hidden out of sight is a big barrel of pain waiting for anyone who dares attack. :)
