I would just like to thank you for this wonderful script. :)

It works even for addresses with the library proxy string at the end. Just add the proxy-string attached pubmed URL to this script.
Right click on greasemonkey icon > Manage User Scripts > Post Entrez Pubmed Citations to CiteULike > Add library proxy-string attached pubmed address under "Include Pages" for eg. http://www.ncbi.nlm.nih.gov.ezp.miner.rochester...*

NCBI changed the URLs for papers to
http://www.ncbi.nlm.nih.gov/sites/entrez? ...
from
http://www.ncbi.nlm.nih.gov/entrez? ...

I've updated the script to handle the new URLs as well as the old ones.
