ITA Polish v1.4

Adds some handy features to the matrix.itasoftware.com flight itinerary search tool.

This script inserts some handy features into ITA's search and results pages. So far the changes include:

- Properly setting default departure city and sales city
- Adding airport code quick reference (via airplane icon) to search pages
- Adding webflyer links to search results, to calculate mileage.
- Adding a help link for advanced query composition to the action bar at the top-right of search pages
- Adding several custom options to the 30-day window search
