Adds a favorites link (star icon) to header of Youtube pages for quick access to your favorites.

Updated again to work with Youtube's latest header.
Removed some fallback code as Youtube's Upload page now has their latest header on all pages. The Uploads page used to have their old code.
Cleaned up the code as it's now simpler than ever.
Added an author info in the header.
